This is the lodging form for a twin-sharing room in Cititel Mid Valley.

1. Each booking consists of a twin-sharing room, with breakfast for 2 people.

2. The discounted rates are only applicable to bookings through the organizers.

3. Lodging rates on/before 30th June are 50 USD per room per night. Lodging rates after 30th June are 55 USD per room per night.

4. All international guests are required to pay the hotel a surcharge of RM10 tourism tax per room per night, not excluding other fees charged by the hotel.

5. Bookings will be confirmed upon completion of payment.
